Insurance coverage is an important condition for a test to become more widely used. But increasing the number of people who can access a test is not the same as increasing the number who actually take it.

On September 14, Guardant Health announced that its Shield blood test for colorectal cancer had been included in an EviCore by Evernorth policy, effective January 1, 2027. This is a change in insurance coverage, not a report of improved detection performance in a new clinical trial. Guardant Health announcement ↗

That distinction matters when assessing the business. A healthcare provider must offer the test under an eligible insurance plan, the patient must take it, and the insurer must actually pay. There are several steps between a broader policy and revenue. A company's stated eligible population cannot simply be treated as a forecast of future test volumes.

Another point to follow is what happens after a positive result. Shield does not complete the diagnostic process with a blood draw alone; a positive result requires further evaluation by colonoscopy. Intended use and follow-up after a positive result ↗

If a blood draw helps people who have not previously been screened make it all the way to diagnosis, wider adoption of the test could have considerable value. Its medical value would look different, however, if it mainly replaces other screening methods among people already being screened, or if many people cannot proceed to follow-up testing after a positive result. Rising test volumes alone cannot tell us this.

This card will track the policy's effective date, actual test volumes and payments, participation by previously unscreened people, and completion of colonoscopy after a positive result as separate measures. Expanded coverage puts one condition for growth in place. The next thing to watch is how far that opportunity translates into diagnostic and business results.

Two same-quarter observations a year apart; the path between them is unknown. This does not measure the effect of the September Carelon announcement. Ratio calculated from rounded disclosures.

Track implementation, tests and payments, participation by previously unscreened people, and completed follow-up colonoscopies separately.

Hypothesis and what would change it

Broader coverage combined with follow-up support may increase the number reaching diagnosis, not just the number tested.

What would change our view: Reconsider if wider eligibility does not increase new screening or completed follow-up colonoscopies.

A planned change reported by the company. The underlying policy and individual plan conditions have not been independently checked; this does not establish unconditional coverage or increased revenue.
